New Holland 7840 with Loader Blue Diecast Model Tractor Loose.
 
I have seen multiples of these sold by ERTL in different packages labeled 1997, 2000, 2001 and am not sure which set it came from. There is also one that was produced for a Farm Show in 1989 that was limited to 200 pieces. This one is not stamped with the ERTL logo anywhere I can see. The only markings (other than the branding decals) is a stamp on the bottom that says "0862Q Made in China" on the bottom. I searched this with New Holland and didn't find anything.
